GLP-1 Cost Without Insurance: 2026 Prices

If you are paying for a GLP-1 yourself, do not start with the list price. In 2026, manufacturer self-pay programs can put the price well below list price — but the best route depends on your drug, dose and insurance.

If you have Medicare, check coverage before paying cash: eligible Part D beneficiaries can now get certain weight-management GLP-1s for $50 a month through the Medicare GLP-1 Bridge.

For everyone else, compare your insurance price, manufacturer program and pharmacy cash price.

These are program prices, not universal cash prices. Eligibility and terms apply, and manufacturers can change them.

What is the cheapest order to check?

Start with the payment path that could beat cash.

  1. Medicare Part D: Check whether your prescription belongs under regular Part D or the Medicare GLP-1 Bridge before paying cash.
  2. Medicaid: Check your state program and plan. Coverage for weight-management GLP-1s varies, and the federal BALANCE model is voluntary for states.
  3. Commercial insurance: Ask for your exact patient responsibility, then check whether a manufacturer savings card lowers it.
  4. No useful insurance coverage: Compare the manufacturer self-pay price with the pharmacy cash price.

That order matters because paying outside insurance can mean the purchase does not count toward your deductible or out-of-pocket maximum.

Why can the advertised price be misleading?

Because "starting at $149" or "$199 for Wegovy" is not one permanent price for every patient and dose.

Foundayo pricing changes by dose. Wegovy and Ozempic have limited-time starter offers for eligible new patients. Zepbound's higher-dose promotional pricing can depend on refilling within a stated window.

The useful question is not:

What is the advertised GLP-1 price?

It is:

What will my exact prescription cost this month, and what does that payment do to my insurance benefits?

What if insurance says the drug is covered?

"Covered" still does not tell you what you will pay.

Ask your insurer or pharmacy for your patient responsibility after any deductible, copay or coinsurance. Then compare that amount with the manufacturer price.

Suppose your insurance price is $430 and a self-pay option is $349. Cash saves $81 today. But if the $430 would count toward your deductible and the $349 would not, you have a second tradeoff to consider.

What should I ask before I fill the prescription?

You can usually get what you need with four questions:

  • What will I pay if you run this through my insurance?
  • Is there a manufacturer savings program for my exact prescription?
  • What is the manufacturer self-pay price for this dose?
  • If I pay outside insurance, will it count toward my deductible or out-of-pocket maximum?

If you have Medicare, use the 2026 GLP-1 insurance guide before paying cash. The new federal rules can produce a very different price.

Bottom line

There is no single "GLP-1 cash price."

In 2026, the lowest legitimate price may come from your insurance, a manufacturer savings card, a manufacturer self-pay program or — for eligible Medicare patients — the $50 Medicare GLP-1 Bridge.
