LillyDirect vs NovoCare: GLP-1 Prices Compared

LillyDirect and NovoCare are manufacturer-backed ways to get certain GLP-1 prescriptions, but neither is automatically the cheapest option.

LillyDirect is Eli Lilly's platform. NovoCare is Novo Nordisk's. Both can offer lower self-pay prices than list price, but the drugs, dose pricing and insurance rules differ.

The simplest approach: start with the drug you were prescribed, then compare your insurance price with the manufacturer's price.

Which GLP-1s can you get through each program?

Those numbers are not interchangeable. Foundayo, Wegovy and Zepbound have weight-management indications. Ozempic and Mounjaro are diabetes medicines with different FDA-approved uses.

Your prescriber decides which medicine is appropriate. The shopping decision starts after you know what was prescribed.

Does manufacturer-direct mean you skip insurance?

No.

LillyDirect can support insurance and self-pay pathways depending on the medicine. NovoCare also offers insurance savings programs, while its NovoCare Pharmacy cash/self-pay transactions are processed outside insurance.

That creates two very different routes:

Insurance route: Your plan processes the prescription. A manufacturer savings card may reduce your remaining patient responsibility if you qualify.

Self-pay route: You pay a manufacturer-backed cash price outside insurance.

If you use the self-pay route, do not assume the payment will count toward your insurance deductible or out-of-pocket maximum.

What changed for Medicare in 2026?

This is where the comparison changes substantially.

Starting July 1, 2026, the Medicare GLP-1 Bridge gives eligible people with Part D access to:

  • Foundayo tablets
  • Wegovy injection or tablets
  • Zepbound KwikPen

for a $50 monthly copay.

That means an eligible Medicare patient should generally check the Bridge or regular Part D coverage before choosing LillyDirect or NovoCare self-pay.

The Bridge is not normal Part D coverage. Its $50 copay does not count toward the Part D deductible or true out-of-pocket costs.

Medicare also says patients prescribed a GLP-1 for a Part D-coverable indication — such as Zepbound for moderate-to-severe obstructive sleep apnea or Wegovy for certain cardiovascular-risk reduction — should use the regular Part D pathway rather than the Bridge.

Which program is cheaper?

There is no useful "LillyDirect vs NovoCare winner" because the programs usually sell different prescriptions.

Compare this instead:

Your prescription → insurance price → manufacturer savings offer → manufacturer self-pay price → pharmacy cash price

For example, NovoCare's lowest Wegovy pill price does not make Wegovy the "cheapest" choice for someone prescribed Zepbound. Changing medication is a clinical decision.

What are the catches to check?

Manufacturer prices often come with conditions:

  • A price may apply only to certain doses.
  • A low introductory price may last only a few fills.
  • A higher-dose discount may require refilling within a set number of days.
  • Commercial savings cards may exclude government-insured patients.
  • Self-pay purchases may not count toward insurance cost-sharing totals.

The headline number is only useful after you know which version of the offer applies to you.

What should I do before paying?

Ask for three numbers:

  1. Your actual insurance patient responsibility
  2. The manufacturer price for your exact drug and dose
  3. The pharmacy cash price

Then ask whether choosing the manufacturer self-pay price takes the prescription outside your insurance benefits.

Bottom line

LillyDirect and NovoCare make GLP-1 pricing more visible. That is useful.

But manufacturer-direct is another price to compare, not a reason to automatically bypass insurance.

For Medicare patients especially, check the new 2026 federal coverage pathways first.
